Submitting the Admissions Conduct Form
For applicants who apply online, the Admission Conduct Code question is part of the online application process. You will be prompted to provide the necessary information and it will be submitted electronically. There is no need to follow-up with paper copies.
All applicants must submit the Admissions Conduct Form. If applying with a paper application, this form must be sent directly to Rackham Graduate School Admissions. Do NOT send it to the program of study.
