A former undergraduate from the University of Michigan Ann Arbor campus changes their legal name after graduating. They plan on applying to a master’s program within Rackham. We advise that they contact the Registrar’s Office before submitting their application for admission, and request that their legal name be changed in their Ann Arbor campus records to ensure that the name entered in the application matches.

Download the HR30005 Address/Personal Data Form and follow the directions for completing and submitting the form and associated legal documents. Please note, personal information should never be sent via email. All HR forms and associated legal documentation must be mailed or faxed to the Shared Services Center following the instructions at the bottom of the form.

If You Are an International Citizen Holding a Non-Immigrant Visa
Consult with the office that issued the immigration documents for the non-immigrant visa to determine if the change in personal information will impact your immigration documents before changing personal information. Changing the personal information in your University of Michigan record may require requesting new immigration documents.
